LTPAC Transitions Program
As the state designated Health Information Exchange (HIE), CORHIO was awarded a grant from the Office of the National Coordinator for Health Information Technology (ONC) to facilitate adoption and measure the impact of HIE on long-term and post-acute care (LTPAC) transitions.
While all LTPAC organizations in Colorado are eligible to participate in the CORHIO HIE, CORHIO currently is offering compensation from the ONC grant to eligible LTPAC organizations with operations in Boulder County, Colorado Springs, Pueblo and the San Luis Valley to adopt HIE and assist in CORHIO's LTPAC Transitions Program. If your organization operates in one of the four target communities, but also has facilities located in other communities, you may still apply for the LTPAC Transitions Program. Compensation for participation will be determined based on a pro-rated calculation for facilities located in the four communities only.
Participating LTPAC organizations will receive training and access to the CORHIO HIE, via CORHIO's PatientCare 360°™ Web portal. Participants will be requested to attend local community-level care transitions meetings and complete several assessments and surveys over the course of the program.
Program Participation Eligibility Criteria
- Meet one or more of the following participant types:
Home health agency
Hospice
Skilled nursing facility
Assisted living facility
Long-term acute care (LTAC)
Residential care facility for the developmentally disabled (RCF-DD)
- Have existing operations in Boulder County, Colorado Springs, Pueblo and/or the San Luis Valley.
- Currently receive referrals from hospitals connected to the CORHIO HIE.
- Agree to complete and submit required program assessments and surveys.
- Submit a fully completed application. Please note that incomplete applications will not be accepted for consideration until fully completed.
