PatientCare 360 Users – Did you know you can access care summaries from these organizations?

You can access recent care summaries on your patients from Kaiser Permanente, Department of Defense, Veteran’s Administration and Quality Health Network.

CORHIO has established partnerships with these four organizations and has completed the technical connections required to make care summary documents, including Continuity of Care Documents (CCDs), available for download to participating providers on the CORHIO network. These care summaries of recent visits ensure continuity of care as patients move in and out of health systems and care networks throughout the state.

Using the CORHIO secure web-based PatientCare 360 portal, a provider can easily find hospital records, clinical documents, test results, and more information to care for patients who may have traveled to another location to receive care or were out of town when a health incident occurred.
