Kaiser Permanente Colorado Data Now Available in CORHIO Health Information Exchange

Care summary documents for patients affiliated with Kaiser Permanente Colorado providers are now available to community healthcare providers via the CORHIO PatientCare 360® web portal.
Kaiser Permanente Colorado has enhanced their participation in CORHIO’s health information exchange (HIE) network and is now making care summary documents available to the community via CORHIO’s web portal, PatientCare 360®. The care summary documents include clinical information on patients recently seen by Kaiser Permanente Colorado physicians.

Vital Clinical Information at the Point of Care
As with all care summary documents, the documents from Kaiser Permanente Colorado include several pieces of clinical information that can be very helpful to providers outside of the Kaiser network. In particular, this information is useful for emergency department providers and other clinicians who see Kaiser Permanente Colorado patients, and want a faster way to access their data than requesting it be faxed.
The care summaries from Kaiser Permanente Colorado include the following information, if it’s available for the patient:
- Active allergies and adverse reactions
- Active and resolved problems
- Advanced directives
- Care plans
- Current medications
- Encounters and procedures
- Immunizations
- Insurance
- Notes
- Social history
- Test results
