Funding Opportunity for Medical-Dental Integration Project

The second wave of the Colorado Medical-Dental Integration Project is now open for applications. This project from Delta Dental of Colorado Foundation and The Colorado Health Foundation aims to integrate medical and dental care and to reach Coloradans with limited access to dental care due to location, insurance status and transportation issues. Grant funding is now available for nonprofit primary care clinics and school-based clinics to integrate a hygienist into their practice.

Funding will be considered for:

  • Federally Qualified Health Centers integrating a dental hygienist as part of the medical team
  • Nonprofit safety-net practices integrating a dental hygienist as part of the medical team
  • Dental practices collaborating with medical practices to partner as dental homes

Funding priority will be given to organizations that:

  • Have existing or evolving relationships with community dentists to refer to when patients need exams and restorative dental care
  • Provide a medical home to children and/or adults experiencing oral health disparities
  • Demonstrate a clear desire to expand patient services to include full scope preventive dental care
  • Demonstrate a readiness for medical-dental integration as defined in the CO MDI toolkit and through the toolkit’s readiness assessment
