COVID-19 UPDATE: Our COVID-19 Lab Notifications service that brings real-time alerts of test results to providers is now available at no cost to qualified participants.  

 

Through real-time or daily notifications, we can notify you when there is new information on your patients or members. Simply provide us with a current patient list or a targeted subset of patients and we return to you the most current data available from our network of participating labs, hospitals and health systems.

Through an electronic report or HL7 interface, we can notify you when patients are hospitalized or visit an emergency department or when new lab results are available (for Diabetes, Cholesterol and Hepatitis at this time). This timely information can be used to improve care management for patients, prevent hospital readmissions, improve disease management programs and inform quality improvement programs.

Utilize Notifications to:

  • Save your care managers time in coordinating patient care, such as follow-up treatment or medication review 
  • Increase Medicare reimbursements by utilizing TCM (Transitional Care Management) codes for timely patient follow-up (up to $230)
  • Review more detailed records from your patient’s recent encounter via our secure web-based portal PatientCare 360
